Remote Patient Monitoring (RPM)

Relies on devices to transmit health data outside of a clinical setting. RPM services allow the healthcare provider to monitor a patient’s blood pressure health on an ongoing basis in between visits. Remote Patient Monitoring is serviced through Synergy Care Management and will complement the healthcare provider assessments to identify issues that need to be addressed sooner and allows patients to communicate their monitored health information to their provider without having to travel to your provider’s office. When patients are seen for their regular scheduled appointments, the readings our device provides will help the healthcare provider make treatment recommendations.
