Principal Care Management (PCM) Patient can have 1 Chronic Condition to be eligible (Same as with CCM Conditions) Requires 30 minutes per month to be billable (Medicare)

Principal Care Management PCM:

• A patient with a qualifying, complex condition that is expected to last between three months and one year, or until the death of the patient, may have led to a recent hospitalization, and/or place the patient at significant risk of death, acute exacerbation/decompensation, or functional decline. 
• Program specific, patient’s health information is intrinsically recorded with Certified EHR (Electronic Health Records) Technology, including all updated demographics, health concerns, medications and medication allergies, and personal diagnostic measures that inform the care plan, care coordination, and ongoing clinical care
